Referrals
Information for patients, family physicians, nurse practitioners, and other healthcare providers.
For patients
In Ontario, specialist psychiatric care generally requires referral through a family physician, nurse practitioner, or appropriate healthcare provider. Self-referral is not typically available.
Please ask your primary care provider to submit a referral on your behalf.
For referring clinicians
Referrals should ideally include:
- Presenting concern and reason for referral.
- Psychiatric and medical history.
- Current and past medications.
- Relevant investigations.
- Substance use history, if relevant.
- Risk concerns, if relevant.
- Patient contact details and pharmacy details.
Referral review and triage
Referrals are reviewed and triaged. Appointment length depends on complexity and clinical need. Reception will contact the patient to confirm scheduling and pharmacy details.
