What to expect

  • A referral is required from your family physician or nurse practitioner.
  • Appointments are usually about two hours; some are one to four hours depending on complexity.
  • Breaks and environmental adjustments are available.
  • A support person may attend if you wish.
  • A collaborative treatment plan is developed with you.

What to bring

  • Valid OHIP health card.
  • List of current and past medications.
  • Contact details for your family physician.
  • Insurance information, where relevant.
  • Notes about your symptoms, concerns, and treatment goals.
